I would like to file a formal complaint to (Name Here), (Position Here), regarding a breech in confidentiality concerning the state of my health.

It has been brought to my attention that the aforementioned name has explicitly revealed the true nature of my health to an individual who has no professional connection to this distinguished institution.

I was expecting that this sensitive matter, my health and its present status, would not leave the bounds of this establishment. Unfortunately, it already did.

In a few days time, I will be passing this letter to the nursing office against an individual I truly admire, in relation to work.

You know who you are. What you did was seriously below the belt. If we were back in the Philippines, I'd file this complaint to the Professional Regulatory Commission, Board of Nursing (PRC-BON) to revoke your license.

Sheesh. What a low life.

This has to end. And, unfortunately, I will be the person who will end it. Just wait and see.
