- Photocopy of updated PRC License

- Photocopy of updated IV therapy license (2 copies)
- Photocopy of IV therapy certificate (2 copies)
- Photocopy of PNA (Philippine Nurses Association) ID
- Photocopy of BLS (Basic Life Support) certificate
- Photocopy of trainings and seminars attended
- Photocopy of 3 recommendation letters from previous employer or college dean, if fresh graduate
- Photocopy of employment certificate or clearance from previous employer

Hey Guys and Juan Paolo, I need your help..I am scheduled for a panel interview in PHC next week..I dont know how to prepare and I dont know to expect sa interview..I've waited for this opportunity for more than 1 year, kaya ayaw ko sana palampasin. So, any feedback and tips lalo sa mga na-interview na and staff na sa PHC..How about my friend, she is 2nd priority, is there a chance for her to get in PHC? Thanx..take care always and God bless..

Yeah, I think so. I just attended a job fair at Market! Market! and they told me that as long as you have your complete requirements, you'll be still entertained. And the usherette told me they're really strict with the requirements so better not pass them until they're not. :)
